Community Stroke & Rehabilitation Center is committed to providing patients and their loved ones the best compassionate care, experience and treatment in everything we do.

Any restrictions are imposed only for the benefit of the patients. Visitors may be asked to leave at any time if the patient chooses, staff must provide care at the bedside or if it is in the best interest of the patient. Please do not visit if you are ill or experiencing symptoms of illness that may make you contagious.

Community Stroke & Rehabilitation Center in Crown Point is a four story, 129,000 square foot multispecialty hospital. The facility features a 40-bed inpatient rehabilitation unit. In addition, a wide range of outpatient services include:

  • Immediate care
  • Physical, occupational and speech outpatient therapies
  • Diagnostic imaging: MRI, CT, X-ray, ultrasound, 3D mammography, bone density, pulmonary function testing, EEG, EMG, echocardiography, EKG and nuclear cardiology
  • Laboratory services
  • Women’s Diagnostic Center
  • Physician specialties in cardiology, family medicine, gastroenterology, internal medicine, neurology, obstetrics/gynecology, orthopedics, pediatric medicine and urology
  • Valori Kolarczyk Healing Garden

At Community Stroke & Rehabilitation Center, the Valori Kolarczyk Healing Garden was established outside the Vista Café patio as a place for reflection, rehabilitation and gathering. Out in the garden, patients can tap into all of their senses and draw in the healing qualities of nature. Community Healthcare System’s medical professionals believe healing is a full-body experience and taking care of the mind is a vital part of the recovery process. Valori was the former program director of Acute Rehabilitation at St. Mary Medical Center and was dedicated to helping patients achieve functional recovery from illness, injury or disability. She unfortunately lost her battle with cancer a year before the center was built. The idea of the healing garden was created as a way to continue her legacy and offer patients and visitors a soothing space in which to heal.
